Outlook South West is currently seeking a Senior Psychological Wellbeing Practitioner for the opportunity to work within both the core IAPT team and the Healthy Outlook Team, which is a newly commissioned IAPT service, or to work exclusively in one of the teams.

The objectives of the LTC service are to integrate care with Physical Health Services to improve physical health outcomes for service users with Physical Health conditions and Mental Health conditions.  

A key component of the role will be to support PWPs and Physical and Mental Health Practitioners (PMHPs) with case-management supervision, attend consultation meetings with physical health services and complete consultation assessment meetings with service users who will be directly referred to our team from physical health services. 

The role will require you to offer supervision and support practitioners and provide treatment for service users with our adapted LTPHC treatment programme.  This will include further face to face/telephone reviews, online treatment packages, workbooks and courses. Throughout this time, you will also be responsible for gaining specific measures for outcomes and liaising with the physical health teams to integrate service user care.
