You have the legal right to request a copy of the information we hold about you, in line with the General Data Protection Regulation (GDPR). The information below explains what options are available under a Subject Access Request or requesting a concise report, and any associated fee. It will also tell you what you need to do to progress a request. Under the General Data Protection Regulation (GDPR), the deadline for completion of a Subject Access Request is one calendar month from the receipt of the request and appropriate Identification. Please bear this in mind when making your request. We are able to provide any of the following: A copy of your clinical record under a Subject Access Request which is free of charge. A copy of any letters sent by Outlook South West to your GP to update after assessment and/or discharge under a Subject Access Request which is free of charge. Confirmation of attendance at appointments or a course, not containing any clinical information, which is free of charge. A concise report which will contain reason for referral, sessions attended, the scores from completed questionnaires and their relevance and meaning, confirmation of the outcome of any assessment and a brief narrative by the therapist concerning treatment. The fee for this is £10.00 You will need to let us know which of the above options (or combination of options) you would prefer. Proof of Identification In order for us to process your request we will ask you to provide the following proof of identity: At least TWO types of identity validation. The following two items: Copy of valid passport Copy of current photo driving licence If unable to provide both of the above, we require at least one of the above in addition to one of the following: Copy of birth certificate Copy of paid utility bill (dated within 3 months) Access to Personal Data Form It will assist us if you can provide the request in writing. This can be done via a completed “Access to Personal Data form”. You can obtain an Access to Personal Data form by emailing email@example.com or writing to: Subject Access Requests, Outlook South West, 2D Restormel Estate, Lostwithiel PL22 0HG. You can also call Outlook South West on (01208) 871905 (lines open between 9.00 am and 1.00pm) If the form is not completed, the request can be a letter or email, as a minimum confirming the following: Patient name Patient address Patient date of birth Details of the request (what information is required?) An e email address that we can send your completed request to. Signed consent and ID as above from the patient if they are not the person making the request Payment for Concise Report Payment (£10 for the report) must be made in advance, either by cheque or BACs payment. Outlook South West BACs details will be provided if required when you make your request. All of this information must be provided prior to us providing access to or disclosing personal identifiable information. Unfortunately we will not be able to process your request until we are in possession of all of this information. We will be able to process your request once copies of your ID, your completed Access to Personal Data form, request in writing or verbal request and any appropriate payment have been received.
